Dr. Sidharth Sharma will be retiring from Primary Care of Milton as of December 23, 2024, however, Primary Care of Milton will maintain continuity of your care and provide you with the same excellent care to which you are accustomed.
We will maintain your patient records in the Northside medical record system. If you would like a copy of your medical records transferred to another provider, please submit a signed Medical Records Release form which may be downloaded here: primarycareofmilton.com/patient-resources/forms
You may also obtain the authorization form on your next visit to the office and mail the completed form back to us, or you may request that we provide your records directly to you.
